Family Health Care Center offers a full range of primary care services, providing comprehensive medical care for all members of the family, from infants to the elderly.

 

Because we are committed to the idea that prevention is better than treatment, we also strive to help our patients develop good health habits.

 

Family Health Care Center's Behavioral Health Program is an integral part of our comprehensive health care services to insure that patients and their families receive the best care available.

 

Our services are based on the Chronic Care Model, in which clinician teams at Family HealthCare Center incorporate the community, the health system, self management support, delivery system, decision support, and clinical information systems into the delivery of health care.

 

Self management support is an innovative leap forward from just providing patient education. Individualized health plans, and self management goals, are designed by the patient with assistance from the medical team.

 

The patient takes the leading role and accepts responsibility for achieving those goals.

 

The educational support needed to meet those goals is achieved as the relationship between clinicians and patients develop.

 

Patients are not just told what to do, they receive training in proven methods of prevention and minimizing complications.

 

Planned and Group Visits are an integral part of Family Health Care Center’s chronic care protocols.

 

Instead of waiting until something goes wrong, patients with chronic conditions at Family Health Care Center have regular planned visits that help them stay in control of their condition.

 

Plus, group visits provide the opportunity for patients to receive services in a support environment that also motivates them to achieve better health outcomes.

 

 

FAMILY HEALTH CARE SERVICES

 

  • Family Practice : Full care for the family is offered at our centers.
  • Family Practice and Obstetrics : Our family practice physicians at our centers offer obstetric care as part of their practice.
  • Healthy Beginnings : Complete care for expectant mothers before, during and after pregnancy.
  • Internal Medicine : Internists at our three centers offer care for adult patients with a broad range of medical conditions.
  • Obstetrics and Gynecology : Obstetrical/gynecologic services are available at our centers.
  • Pediatrics : The special needs of our younger patients can be treated at each of our three centers through family practice providers with services of a pediatrician available at our center.
  • School Health Nurse : A school health nurse works in the Hesperia public schools.
  • Teen Health : A Teen Health center is located in the Community Schools with some services provided at our medical center. Programs include health promotion and prevention, acute care, and health education.
  • Women's Health : Full care for women including yearly exams, health screenings, exercise and nutrition information, hormone replacement, bone density referrals, prenatal care, pregnancy testing, STD testing, and family planning provided.
  • Behavioral Health : Our trained professionals can help you resolve emotional and behavioral symptoms effecting your personal, social and professional areas of life.
